Aneurysmal Bone Cyst of the First Sacral Segment Treated by Excision with Combined Anterior and Posterior Approach



Abstract
Aneurysmal bone cysts are uncommon lesion. Especially in the spine. Among the spinal lesions, it's incidence in sacrum were reported very rate (0.6-3%).
The author's case is the lesion of the first sacral segment that involves the body and the left side ala extensively and showed the collapse due the pathological fracture. Neurological involvement was not observed. After arterial embolization.
Complete
excision of tumor was performed through the combined anterior and posterior procedure under the same anesthesia. In first stage operation, the excision of the posterior elements and whole of left side ala portion was done and transpedicular
Cotrel-Dubousset instrumentation was performed between the fifth lumbar vertebra and bilateral iliac tuberosities.
And, in second stage, excision of first sacral body, and insertion of autologous iliac strut grafts between the bodies fifth lumbar vertebra and 2nd sacral segment were done.
Hip spica cast was applied for three months, postoperatively. Solid bony fusion without collapse of grafted bone blocks was observed at the follow up study of 12 months after operation.
